17세가 된 소녀 리건(Regan : 린다 블레어 분)는 자신도 모르게 이상한 힘을 발휘하는 일들을 하게 된다. 추기경(The Cardinal : 폴 헨레이드 분)의 부탁을 받고 필립 라몬드 신부(Father Lamont : 리차드 버튼 분)가 리건의 집으로 온다. 리건은 정신과 의사인 더스킨 박사(Dr. Gene Tuskin : 루이스 플레쳐 분)의 정밀검사를 받는데 모든 것을 과학적으로만 처리하고 악령을 믿지 않는 박사와 신부는 의견 차이를 일으킨다. 리건을 치료하다 죽은 메린 신부(Father Merrin : 막스 본 시도우 분)의 행적을 조사한 라몬드 신부는 리건에게 분명히 악령이 깃들어 있다는 것을 확인한다. 더스킨 박사가 고안한 기계를 이용하여 리건의 기억 속으로 들어간 라몬드 신부는 악마와의 싸움에 뛰어든다. 라몬드 신부는 악마가 리건의 완전한 영적 힘을 얻는 것을 거부한다고 더스킨 박사에게 말하지만 그녀는 신부가 악마에 미쳤다며 믿으려하지 않는다. 악마에 대해 조사하러 아프리카로 갔던 라몬드 신부는 코쿠모(Older Kokumo : 제임스 얼 존스 분)로부터, 각자 혼자 행동하는 낙천적인 곤충 메뚜기가 폭우가 쏟아지면 날개를 비비게 되는데, 이렇게 되면 성격이 변해 한데 뭉쳐지면서 공격적이 된다는 말을 듣는다. 아프리카에서 돌아온 신부는 악마와 싸우기 위해 리건과 워싱톤으로 간다. 더스킨 박사와 샤론은 신부 뒤를 쫓아 오고, 마침내 죠지 타운 프로스텍트 8번지에 도착한 신부는 메뚜기떼에 휩싸여 리건의 모습을 하고 있는 악마와 싸워 마침내 리건을 구해낸다.
